The note is written by Don Carter. In the first part of the note it is written to a hospital staff member criticizing the hospital's infection protocol.
In the second part of the note, Don starts describing his worsening symptoms from his infection and asks for his wife Sasha. The note ends with Don's symptoms progressing to the point where he can no longer write properly, still asking to see his wife.

update. The doctor put some ointment on my bite mark and then vanished. This thing hurts and seems to be getting worse.
